“Successful removal of bile duct stones through ERCP”

Udaipur, Geetanjali Medical College & Hospital’s Gastroenterology Department’s Dr. Pankaj Gupta performed a successful ERCP procedure to extract more than 100 bile duct stones from a 46 years old patient. Besides, it’s the only hospital in Southern Rajasthan to have used this new technique for a patient having around 100 gallstones.Udaipur resident, Gautam Lal, Age 46 years, was suffering from pain in abdomen, jaundice & fever since two months. On a routine consultation at Gastroenterology Department of Geetanjali Medical College & Hospital to Dr. Pankaj Gupta, the clinical investigations of Ultra Sound of abdomen & Endoscopy confirmed the presence of 100 stones in Common Bile Duct (CBD). Due to this the gallbladder was swelled up to 25 millimeters which generally is 6 millimeters in width. This again was suppressing the liver because of which the patient was suffering from jaundice & fever.
“Usually, a General Surgery is planned to remove the gallbladder stones to treat symptomatic gallstones and is generally the treatment of choice when a person is troubled by frequent attacks. But minimally invasive laparoscopic techniques have revolutionized treatment for gallstones. Endoscopic Retrograde Cholangio Pancreatography (ERCP) is a technique used to treat gallbladder disease. It is a nonsurgical procedure and the method of choice to find and remove gallstones that are lodged in the common bile duct. During ERCP, a patient is sedated enough to relax, then swallows an endoscope, a long and flexible lighted tube that is connected to and sends images to a computer and video monitor. It guides the endoscope through the stomach into the small intestine and injects a special dye that allows the visualization of ducts in the biliary system. Once the blocked duct is located, the doctor makes an incision with an instrument inside the endoscope, captures the gallstone in a tiny basket, and then pulls it out through the endoscope. We have conducted the procedure free of cost under the flagship scheme of Rajasthan Government’s Bhamashah Swasthya Bima Yojana (BSBY),” said the treating Dr. Gupta.
“Though treatment of CBD stones and removal of stone less than 20 mm is usually done by ERCP; & in this patient all the stones were between 5 to 10 mm in size, so this was the best optimal solution for treatment. The stones in gallbladder could be life threatening if not timely intervened. Generally only 1 or 2 stones are diagnosed in gallbladder, but in this case more than 100 stones in gallbladder are very rare & a big surprise too”, Dr Pankaj Gupta stated.
